SUPERCHIPS AS SPECIFIED WITHIN 30 DAYS OF DATE OF INITIAL PURCHASE. FAILURE
TO REGISTER PRODUCT WITHIN THIS TIME LIMIT WILL VOID THE LIMITED WARRANTY OFFERED
ON THE PRODUCT CONTAINED HEREIN.

This Registration Form must be completed and received by Superchips by mail, email, fax or Online within 30 days from the date of purchase to activate the Limited Warranty. Original proof of purchase/receipt will be required if a claim is filed. This Limited Warranty is a value added benefit to customers of Superchips and is not for sale or resale to consumers or transferable to a third party.
